In 2006, The California Wellness Foundation (TCWF) commissioned the National Health Foundation (NHF) to conduct a confidential survey of TCWF grantees and those who were declined funding during 2005. NHF's survey results and the survey questionnaire can be accessed above.

This is the fourth Grants Program Survey that TCWF has commissioned since 1997. The survey is part of the Foundation's commitment to learning. Survey feedback is used to enhance TCWF's responsiveness to organizations that seek funding.

Since the release of the 2006 Grants Program Survey, TCWF has had many inquiries for information on the survey. In an effort to give people a better understanding of the type of information collected in this survey, TCWF has made the questionnaire available in PDF format.
